Job Description:
Touchstone Communities, a leader in senior living and long-term
care throughout Texas, is seeking a compassionate and experienced
Licensed Social Worker to join our skilled nursing team. This
position offers a unique opportunity to enrich the lives of
residents by providing tailored care and a supportive environment.
In this role, you will assess resident needs, develop care plans,
and work with interdisciplinary teams to ensure optimal care
delivery. Assess the psychological, social, and emotional needs of
residents and their families. Develop and implement comprehensive
care plans in collaboration with healthcare professionals. Provide
counseling and support to residents and their families. Facilitate
community and resource connections for residents. Evaluate care
plans regularly and adjust as needed. Ensure compliance with state
and federal guidelines. Participate in multidisciplinary team
meetings to discuss resident care and progress. Strong emotional
intelligence and empathy Expertise in geriatric care Excellent
communication and interpersonal skills Knowledge of healthcare laws
and regulations Ability to handle sensitive information
confidentially 401k Licensed Social Worker (LSW) required, Licensed
Clinical Social Worker (LCSW) preferred.
