In this role, you will provide compassionate and efficient nursing
care by visiting patients at their homes. You will work
autonomously to deliver personalized treatment plans and ensure the
health and safety of each patient. This role is pivotal in bridging
the gap between in-hospital care and home recovery, ensuring
continuity of care for patients within the community served by HCA
Healthcare. Assess patient health problems and needs, and develop
nursing care plans. Monitor, record, and report symptoms or changes
in patient's conditions. Maintain accurate, detailed reports and
records of patient care. Administer medication and treatments as
prescribed by a physician. Instruct patients and their families on
proper home health care strategies and procedures. Coordinate with
doctors and other healthcare professionals to plan, manage, and
assess patient treatments. Strong interpersonal and communication
skills Ability to make quick and accurate assessments Proficiency
in documenting and maintaining patient care records Skill in
administering medication and treatments accurately Medical

Valid Registered Nurse (RN) license in the state of practice CPR
certification Driver's license
