Job Description:
AccentCare is on a mission to provide comprehensive and

dedicated Patient Care Manager/Registered Nurse to join our team.
This role offers a unique opportunity to lead and manage patient
care operations while ensuring the delivery of exceptional quality
services. The successful candidate will coordinate and supervise
all aspects of patient care, embodying our commitment to enhancing
the lives of our patients through personalized and compassionate
service. Manage and oversee a team of nurses and healthcare
professionals, ensuring compliance with regulations and quality
standards. Develop, implement, and evaluate individualized patient
care plans based on assessments and consultations. Liaise between
patients, families, and the healthcare team to ensure effective
communication and care delivery. Monitor and ensure the
documentation of patient care is accurate and complies with
industry standards. Administer medicines and treatments ethically
and according to prescribed orders. Facilitate continuing education
and training programs for nursing staff to enhance their
professional skills. Strong leadership and team management skills
Excellent communication and interpersonal skills Proficient in
using healthcare software and systems Adept at problem-solving and
decision-making Comprehensive knowledge of nursing practices and
regulations Medical Insurance Dental Insurance Vision Insurance
401k Valid Registered Nurse (RN) license in the state of employment
CPR certification Certification in Healthcare Management preferred
(e.g., Certified Home Health Care Manager)
