something we bring out in one another. As a PRN Physical Therapist,
you’ll help patients regain mobility, confidence, and independence
where it matters most — in their homes. This role offers the
flexibility of a PRN schedule while giving you the chance to build
meaningful relationships with a diverse patient population. You
will use your clinical expertise to assess, treat, and guide
patients through individualized therapy plans, while contributing
to the strength and clarity of our interdisciplinary team. This is
an ideal opportunity for a PT who values autonomy, patient-centered

collaboration. Key Responsibilities Conduct evaluations,
reassessments, and recertifications in alignment with Medicare,
state licensure, and Touchstone Health standards. Develop and
update individualized plans of care based on clinical judgment and
patient needs. Deliver skilled interventions, home exercise
programs, and education to promote independence, mobility, and
safety. Document care accurately and timely, including OASIS
assessments, progress notes, and ICD-10 coding requirements.
Collaborate with physicians, PTAs, Home Health Aides, and
interdisciplinary team members to ensure aligned care planning.
Provide clear instruction and coaching to patients and caregivers,
adapting communication to each unique home setting. Supervise PTAs
in compliance with state practice acts and organizational policies.
Participate in QAPI initiatives and staff education to support
continuous improvement. What We’re Looking For Licensed Physical
Therapist (PT) in the state of practice; CPR certification
required. Two years of clinical experience preferred — but not
required. We welcome PTs who demonstrate strong character,
willingness to learn, and patient-centered intent. Strong educator
and communicator with deep empathy and respect for patients and
families. Ability to work independently in the field, demonstrating
sound judgment and consistent follow-through. Tech-comfortable and
willing to learn EMR workflows, including OASIS documentation.
Reliable transportation, valid driver's license, and auto insurance

Requirements: Qualifications Graduate of an accredited physical
therapist curriculum approved by the Commission on Accreditation in
Physical Therapy Education, the American Physical Therapy
Association, or the Committee on Allied Health Education and
Accreditation Currently licensed as a Physical Therapist in the
state of practice Two years of clinical experience preferred; home
health experience a plus but not required Ability to exercise sound
clinical judgment and work independently in varied patient care
settings Strong communication and interpersonal skills with
sensitivity to diverse backgrounds Basic computer proficiency;
willingness to learn EMR systems and OASIS documentation Current
CPR certification Commitment to compliance with Medicare/Medicaid
Conditions of Participation, state regulations, and agency policies

for home visits Physical Requirements Must be able to stand, walk,
bend, and assist with mobility or equipment as needed. Requires the
ability to perform heavy lifting and to travel locally in various
