Description: CONTRACT - RN - CASE MGMT (689) - 13 WEEKS - MON-FRI
8:00A-5:00P. NO ON CALL REQUIRED. Responsibilities: Discharge
planning and progression Conducting hard conversations with family,
patients, and open discussion with MD for plans of care
Requirements: Meditech or HCA experience Bedside case management
experience of at least 1 year Post acute care knowledge
