Our motto: To empower people to not simply live a life in recovery but to experience their personal strengths to fuel a better, healthier, more purposeful life.Perks:Benefits (medical, dental, vision, 403(b), PTO, tuition reimbursement, clinical license reimbursement, etc.)Great culture and leadership team POSITION SUMMARY Reporting to the Program Director, the Clinical Assessor is responsible for ensuring that clients are referred to the appropriate level of care by screening, interviewing, scheduling, and completing assessments and completing the intake process with clients and their families both on-site and in the field. ESSENTIAL DUTIES AND RESPONSIBILITIES Prepares daily assessments, screenings, and diagnoses for prompt referral and intake to the appropriate level of care. Managing the admissions line and prompt response to callers utilizing excellent customer service and professionalism. Assisting a potential client that does not need treatment at Phoenix House to connect with outside sources to provide the appropriate resources and care needed. Evaluates test results, psych evaluations, legal histories, social histories, and drug assessments, along with other reports/interviews to determine if the candidate is appropriate for treatment at Phoenix House. Performs primary counselor duties to clients and their families, including but are not limited to: Development of treatment plans for review by a supervisorCounseling services such as crisis intervention; group, individual, family and didactic sessionsConducts initial screenings and comprehensive assessmentsPrepares discharge summary and planningCoordination of treatment services with probation, school, and other referral sources.Multi-lingual counselors may be asked to conduct services in multiple languages to meet the needs of the clients and families. Determines and prepares a substance abuse diagnostic impression for each client after reviewing all materials provided, conducting additional assessments, and completing client/family interviews. Coordinates and completes the admissions of clients with approval by their authorized representatives and/or funding agencies after coordinating with the Assessment & Referral team, Client Care team, Billing team, and Treatment team to ensure that potential clients are screened and authorized for treatment by the appropriate funding source(s). Maintains accurate and timely documentation utilizing web-based and internal databases in accordance with the expectations and standards of Phoenix House policy and procedures, state licensing requirements, and funding/contract requirements. Has on-call responsibilities for referrals, admissions, prospective or current patient issues, and needs. Establishes and maintains community agency communications and contacts. Assists in verifying benefits and obtaining authorizations for treatment. Works on a multidisciplinary team to facilitate client services. Attends weekly clinical supervision and reviews with the clinical team, including other counselors and the Clinical Manager as needed. Adheres to the professional ethics and conduct of standards as established by professional licensure and the policies of Phoenix House. Attends all required staff training sessions in accordance with Phoenix House and state licensing requirements, completes all training on time Attends and participates in program and all staff meetings Adheres to and promotes the Phoenix House Mission, Vision, and Values while acting as a role model for others Other duties as assigned EDUCATION/EXPERIENCE/CREDENTIALS QCC requirements required (LCDC minimum) LPC, LCSW, or LMFT required LCDC required, new hires without licensure will be allowed 12 months to obtain this licensure One or more years of counseling experience which include providing assessments or admissions for substance abuse or mental health services Must maintain a valid Drivers' License, have proof of automobile insurance and maintain a safe driving record based on Phoenix House policy KNOWLEDGE, SKILLS, AND ABILITIES Demonstrated commitment and adherence to Phoenix House Mission, Vision, and Values: I CARE For?
