Activities Director- Chosen Healthcare - -Job Summary:The Activity Director is responsible for planning, organizing and implementing an ongoing program of group and individual resident activities designed to meet, in accordance with the comprehensive assessment, the interests and the physical, mental, and psychosocial well-being of each Resident. - - -Description:Under the supervision of the Administrator, the Activity Director is responsible for planning, organizing, and implementing the Facility activity program. Programs shall be scheduled on a daily basis and shall meet the needs and interests of all residents, in compliance with State and Federal Regulations. The Activity Director must provide for an on-going program of activities designed to meet, in accordance with the Comprehensive Assessment, the interests and the physical, mental, and psychosocial well-being of each resident. - Activities shall be available on evenings, weekends and in recognition of holidays, in accordance with State and Federal Regulations. The programs are a coordinated team effort with related Facility services and staff. Programs are to include activities for all functional levels (ambulatory, non-ambulatory, and bedridden residents), and the activities are to be planned for group and individual participation. -Qualifications:

  • Must have High School Diploma or Equivalent
  • Must be a qualified therapeutic recreation specialist or an activities professional who:
  • Is licensed or registered, by the State in which practicing; and
  • Is eligible for certification as a therapeutic recreation specialist or as an activities professional by a recognized accrediting body on or after October 1, 1990; or
  • Has two (2) years of experience in a social or recreational program, approved by the state within the last five years, one of which was full-time in a Resident activities program in a health care setting; or
  • Is a qualified occupational therapist or occupational therapy assistant; or
  • Has satisfactorily completed, or will complete within six (6) months, a ninety (90) hour training course approved by the division and has at least a high school diploma or its equivalent
  • Satisfactory OIG exclusions and background check
  • Satisfactory personal & professional references -PRE-EMPLOYMENT REQUIREMENTS:
    • Satisfactory OIG exclusions and background check
    • Successfully complete a drug screening
    • Must have received, or be willing to receive, the COVID-19 vaccination by date of hire. - Proof of vaccination or exemption is required
